I,                                     (print name) hereby acknowledge the following policies and agree to abide by them during the entire time I am fostering cats for Cornish Rex Friends.  (Please initial on each line)

1.                   I understand that my role is solely as the temporary home, and that the placement of any cats I foster will be conducted by the Rescue Representative.  It is the Rescue Representatives job to review and interview potential adopters.

2.                   I understand that I am personally responsible for any and all financial expenses that I incur in my efforts to foster cats for Cornish Rex Friends.  Although Cornish Rex Friends may reimburse my expenses, I agree that each expense MUST be supported by receipts, MUST be pre-approved by the Rescue Representative, and any expense over $150 for a single cat must also be pre-approved by the Board of Directors.  I accept full responsibility for any expenses incurred by me that fall outside of approved expenditures.  I will always remember that I represent a nonprofit organization and in no way can profit from any activity related to the organization.

3.                   I understand that Cornish Rex Friends cannot guarantee or be held responsible for the temperament, behavior, or health of foster cats that I may handle.  I am aware that foster cats can cause damage to my personal property, other pets, and humans.  I will keep cats securely contained at all times while in my care.

4.                   I understand that it is MY decision to foster cats for Cornish Rex Friends.  I will not hold Cornish Rex Friends liable for ANY damage, injury or harm caused directly or indirectly through my fostering activities with Cornish Rex Friends.  

5.                   I understand that my foster volunteer position will be terminated if my conduct is deemed in opposition of the mission of Cornish Rex Friends, or detrimental to the best interests of Cornish Rex Friends. 

I understand and agree to all of the above, and support the mission of Cornish Rex Friends.  I understand that this form must be received, approved and acknowledged by Cornish Rex Friends before I may do any volunteer work for the organization, and that Cornish Rex Friends reserves the right to refuse my application for any reason.
